Advanced Decisions (also known as Living Wills)

An "Advance Decisions" is a documents you sign while you still have mental capacity that stipulate that you do not want to have certain life sustaining treatments if you should develop certain medical conditions in the future, and at that time you do not have the mental capacity to inform your doctors of your wishes.
